Stone's Company (K) of the 2nd Regiment of Mississippi volunteers, born in Grainger County, the State of Tennessee, aged 35 years, five feet, eight inches high, dark complexion, dark? hair (can't read next several words) profession a carpenter.  Was enlisted by me at Iuka, Miss on the 9th day of April 1861 to serve one year.  And is now entitled to a discharge by reason of disability.
The said Thomas Johnson is entitled to pay and clothing from 30th day April 1861 to the present time having received nothing.
Given in duplicate at Winchester, Va. this 13th day of July 1861.
